That single number, tracked by the World Bank, places the United States below Denmark, below the United Kingdom's historical average, and closer to the demographic profile of Poland than to the high-fertility America of the 1990s. The U.S. has spent two decades quietly converging with the low-fertility norms of Western Europe, and almost nobody outside demography circles has noticed.
The reason this matters is not moral panic about "fewer babies." It is arithmetic. A country's entire fiscal architecture — Social Security, Medicare, the labor force that funds both — was built on the assumption of a growing base of working-age taxpayers beneath a smaller cohort of retirees. That pyramid is inverting. The analytical question for the United States is not whether fertility will keep falling — it has fallen every year but one since 2007 — but how fast the support ratio breaks, and what policy tools, if any, can slow the descent before the math becomes unforgiving.
The Data
The core figure is stark and well-sourced. According to the World Bank's 2024 fertility dataset, the U.S. total fertility rate (TFR) stands at 1.6265 births per woman, translating into a stress score of 43.0 on The Human Index's composite scale — moderate-to-elevated, and trending in the wrong direction. For context, the replacement-level TFR — the rate at which a population exactly replaces itself generation to generation, absent migration — is 2.1 in low-mortality developed economies. The U.S. has been below that line continuously since 2007, according to the CDC's National Center for Health Statistics (NCHS), which tracks the same decline through its own vital-statistics birth data and shows the general fertility rate falling from 69.3 births per 1,000 women aged 15–44 in 2007 to roughly 54 by 2023.
Two adjacent indicators in the current dataset sharpen the picture rather than complicate it. The U.S. divorce rate sits at 2.3 per 1,000 population (UN Statistics / national vital statistics, 2023), which scores a comparatively low stress value of 22.9 — notable because it means the fertility decline is not primarily a story of family dissolution. Divorce rates have actually fallen from their 1980s peak. Income inequality of this magnitude, among the highest in the OECD club of wealthy nations, is one of the most consistently cited economic correlates of delayed and foregone childbearing in the peer-reviewed fertility literature, because it raises the perceived cost of "doing parenthood right" even as median wages stagnate relative to housing and childcare costs.
Put together, the United States' meta-index shows economic stress at 31.3 and social stress at 47.0 — a combination consistent with a population that is not in acute economic crisis but is under a slow-burning structural strain that shows up specifically in household-formation decisions rather than in headline unemployment or inflation figures.
Context
The proximate causes of America's fertility decline are well documented, and none of them is mysterious.
Delayed family formation. The median age of first-time mothers in the U.S. rose from 24.9 in 2000 to over 27.5 by the early 2020s, per NCHS data — a shift driven by extended education, delayed marriage, and career establishment. Delay compounds: each year of postponement mechanically reduces the total window for childbearing and increases the share of women who intended to have children but ultimately have fewer, or none, due to fertility decline with age or simply running out of runway.
Cost of children, especially housing and childcare. The U.S. is unusual among wealthy nations for the near-total absence of federal paid parental leave and heavily privatized, expensive childcare — average infant care now exceeds $11,000 a year in many states, according to Child Care Aware of America estimates, comparable to in-state public university tuition. Contrast this with France or the Nordic countries, which combine subsidized childcare with paid leave and maintain fertility rates modestly higher than the U.S. despite similarly high female labor-force participation. The comparison matters: it shows fertility decline is not simply a function of women working, but of the interaction between work and unsupported caregiving costs.
Inequality and precarity. The Gini score of 56.0 stress noted above is not incidental. Research summarized by the Pew Research Center and consistent with World Bank inequality data shows that Americans increasingly cite financial insecurity, not lack of desire, as the top reason for having fewer children than they consider ideal. Surveys by Pew (2023–2024) found roughly half of non-parent adults under 50 who don't plan to have children cite "just don't want to" alongside a substantial minority citing cost and instability directly.
A generational, not cyclical, shift. Unlike past fertility dips tied to recessions (which typically rebound), the 2007-onward U.S. decline continued through the 2010s recovery and accelerated after 2020, suggesting structural rather than purely cyclical drivers — a pattern the OECD has flagged across nearly all member states, with the U.S. now tracking closer to Southern and Eastern European fertility profiles than to its own historical baseline.
None of this is unique to America — it is the near-universal signature of post-industrial, high-cost, high-inequality societies. What distinguishes the U.S. case is that it is happening inside a country whose entitlement system, immigration politics, and healthcare cost structure are all uniquely unprepared for it compared with European peers that built demographic contingencies into pension design decades ago.
Implications
The consequences sort into three overlapping tracks.
Fiscal. The Social Security Trustees' own annual reports — not an alarmist outside source — project the combined trust funds will be depleted by the mid-2030s, after which incoming payroll taxes would cover only about 80% of scheduled benefits. A shrinking ratio of working-age contributors to retirees, driven directly by decades of sub-replacement fertility working its way up the age pyramid, is the single largest structural input to that projection. Medicare faces a parallel, arguably steeper, version of the same math given healthcare cost inflation running above wage growth.
Labor force and growth. GDP growth is, in the simplest accounting, a function of labor-force growth plus productivity growth. With native-born fertility below replacement, the U.S. labor force's future growth is now almost entirely dependent on immigration — a politically volatile input rather than a stable demographic one. The Congressional Budget Office has repeatedly noted that immigration assumptions are now the single largest swing factor in long-run U.S. growth and deficit projections, precisely because domestic births can no longer be relied upon to supply the workforce.
Social and household structure. A lower-fertility, higher-inequality America is also one with smaller kinship networks — fewer siblings, cousins, and extended family available for informal eldercare and child-rearing support, which in turn raises the market cost of care that used to be partly informal. - Immigration policy shifts — net migration levels are now the primary lever on U.S. working-age population growth; any sustained restriction would accelerate the labor-force contraction the fertility data already implies.
- The 2033-2035 Social Security trust fund depletion date in the annual Trustees Report — each year's report either pushes this date out (good sign, reflecting stronger-than-expected labor force or wage growth) or pulls it forward.
- State-level childcare and paid-leave legislation — a growing number of states (e.g., expansions in Minnesota, Colorado, and New Mexico) are piloting the kind of subsidized-care policy that correlates with modestly higher fertility in European comparators; a federal version would be the single clearest policy signal of an attempt to bend the curve.
- Gini index trend in future World Bank releases — because inequality and fertility are tightly linked in the U.S. data, a meaningful decline in the 41.8 figure would be a leading indicator worth tracking alongside the birth data itself, not after it.
